Directions to Unihealth

USHMC is conveniently located in Southwoods Ecocentrum Business Park, accessible via major routes.

From Central Biñan

Take A. Bonifacio St., turn right onto Maharlika Hwy, turn left onto San Francisco Rd., head straight towards Southwoods Mall, continue onto Enterprise Ave., then at the roundabout, take the 3rd exit. USHMC will be on the right.

From Laguna's capital, Sta. Cruz

Take Provincial Road and Bagumbayan Road to National Hwy, then follow National Hwy and Manila S Rd to Bucal Bypass Rd in Calamba, turn left onto Buca Bypass Rd, then continue on SLEX to Biñan. Take exit 31, continue on San Francisco Rd. Take Enterprise Ave., then at the roundabout, take the 3rd exit. USHMC will be on the right.

From Laguna's biggest city, Calamba

Get on SLEX from Bacnotan Rd/Chipeco Ave and Real Rd, follow SLEX to San Francisco Rd in Biñan. Take exit 31, continue on San Francisco Rd. Take Enterprise Ave., then at the roundabout, take the 3rd exit. USHMC will be on the right.

Patients may be visited during 9:00 AM to 9:00 PM. Only three (3) visitors are allowed per patient.

For your convenience and to make the admission process smoother, please bring a valid ID and your billing address.

Inpatient:

The patient’s relative must personally fill out the request form and submit it to the floor or ward where the patient was admitted.

After submission, please wait for a text message from us regarding the availability of the requested document.

Please take note of the following:

  • The Clinical Abstract and Medical Certificate cannot be released immediately.

  • These documents require a final diagnosis before they can be processed.

  • The preparation of the Clinical Abstract depends on the availability of the ROD (Resident on Duty) or JCON (Junior Consultant), who are responsible for completing the necessary documentation.

Outpatient:

If this patient has already been discharged or is being discharged on the day of the request, the documents will be available 1 to 2 weeks after the request has been made.

Patient Discharge Billing Process

1. The nursing station will provide the patient chart to Billing. The chart will be verified if it includes a complete final diagnosis and, if applicable, a detailed operative technique for any surgical procedures.

2. Charging units and nursing stations will designate the patient as “clear” in the Hospital System, tagging the account as “May Go Home” and facilitating account closure.

3. Benefits Staff will assess the PhilHealth case rate corresponding to the patient’s final diagnosis/procedures.

4. If the patient is HMO-covered:

Our staff will coordinate details including the Statement of Account (SOA), itemized billing, PhilHealth case rate benefits & the attending physician’s information.

Upon receipt of the Letter of Authorization (LoA), we will furnish the Final Bill alongside essential PhilHealth forms (Eligibility, Claim Signature Form (CSF), SOA).

Then, we will apprise physicians of the professional fee computations provided by the HMO.

If the patient is a cash payee:

Once the PhilHealth case has been assessed and categorized as House/Walk-In, the Billing Staff will calculate and communicate the Attending Physicians’ Professional Fees to them.

However, if it’s a Private cash payee case, physicians stipulate their Professional Fee charge to the patient.

6. Once the final bill is calculated and printed, we will notify the patient’s relative through the nursing station.

7. Ensure PhilHealth forms (Eligibility, CSF, and SOA) are duly completed and signed by the PhilHealth member.

8. Proceed with settlement at the cashier.
