How to Get an LOA for
Your HMO
A Letter of Authorization or LOA is a document issued by your HMO provider allows you receive a specific medical service, such as consultation, diagnostic test, lab test, or medical procedure, at an accredited facility like Unihealth Southwoods Hospital. Most HMO plans require an LOA before services can be provided on a cashless basis.
How To Get Your Consultation Approved:
Before heading to the hospital, prepare the following:
- Your HMO card (physical or digital)
- One valid government-issued ID
- Doctor’s referral or test request (for diagnostic procedures)
- Your HMO provider’s contact number or app (if requesting your LOA directly from your provider)
How To Get Your Consultation Approved:

Important Reminders About Your LOA

- LOAs have an expiry date. Make sure your appointment falls within the validity period.
- For procedures that require fasting, process your LOA one (1) day before your scheduled appointment.
- For patients below 18 years old, a parent or guardian must be present to sign the LOA.
- In most emergency cases, an LOA is not required upfront at accredited hospitals. However, you or your companion should notify your HMO as soon as possible.
