Medical Record Request

The Medical Records Section, located at the 8th floor is where you can request for the following documents for you or your patient-relative:

  • Clinical abstract
  • Discharge Summary
  • Operating Procedure or technique
  • Birth Certificate
  • Medical Certificate
  • Death Certificate
  • Fetal Death Certificate

 

HOW TO CLAIM YOUR MEDICAL RECORDS
  • Get a charge slip from the Medical Records Section (8th floor)
  • Present the charge slip to the cashier (Ground Floor) for payment
  • After payment, go back to Medical Records Section and BRING THE FOLLOWING:
  • Valid ID of the patient
  • Official Receipt for the medical record purchase

 

STILL CONFINED-PATIENTS

For patients who need their Certified True Copy of their medical information while admitted in the hospital:

  • Proceed to Medical Records Section located at 8th floor.
  • Get a charge slip from the Medical Records Section for the early medical record request.
  • Present the charge slip to the cashier (Ground Floor) for payment.
  • Release of the medical information of the patient shall be available depending on the availability of Resident On Duty (ROD)

 

DISCHARGED PATIENTS

Patient or relative of the patient must request the medical record needed.

  • Get a charge slip from the Medical Records Section located at 8th floor.
  • Present the charge slip to the cashier for payment
  • After payment, go back to the Medical Records Section after one (1) week and bring the following requirements:
  • If patient representative, present a valid ID, patient’s original authorization letter, and valid ID of the patient, and official receipt for the medical records purchase.
  • If patient, present a valid ID and official receipt of the medical records purchase

 

Note: The medical information requested shall be released three (3) days prior to discharge date of the patient.

 

IF PATIENT IS UNABLE TO CLAIM

If the patient is unable to claim the requested medical record, the patient representative must bring the following:

  • Original authorization letter of the patient
  • Valid ID of the patient
  • Valid ID of the patient representative

 

FOR PROCESSING OF BIRTH CERTIFICATE

 

FOR MARRIED COUPLES who need to claim the birth certificate of their child.

Please bring the following:

  • Original authorization letter of the patient
  • Valid ID of the patient
  • Valid ID of the patient representative

 

FOR PROCESSING OF BIRTH CERTIFICATE

FOR MARRIED COUPLES

For claiming the birth certificate of their child. Please bring the following:

  • Marriage Certificate (two (2) Photocopy)
  • Valid ID of both parents
  • Provisional Birth Certificate from NICU

 

FOR UNMARRIED NEW MOTHERS AND FATHERS who need to claim the birth certificate of their child. Please bring the following:

  • Community Tax Certificate (CEDULA) of the unmarried person’s parents (MOTHER AND FATHER IF BOTH ARE STILL LIVING)
  • Valid ID of both parents
  • Provisional Birth Certificate from NICU

 

NOTE:

Medical records are released 3 days prior to discharge if:

  1. Medical records are to be released 1 week prior to discharged date
  2. There will be no problem in claiming the medical records if
  • Hospital bills are paid
  • Requirements in claiming of MR are complete when submitted.
